Document meas. approx. 8.50 x 11.25 inches. It is pre-printed and filled out in ink. Document is dated April 1901 and requests a Southern Cross of Honor for Ed T. Howell who enlisted as a Private on August 1, 1862 in Company E, 7th and later the 10th Georgia Infantry and was honorably discharge on April 26, 1865 as a Sergeant.

The document has two horizontal fold lines. The paper is a little weak along the edges of the fold lines but otherwise it is in good condition.
