Ink on 8 ½” x 5 ½” lined, tan paper.  Front reads in part:  “I certify that the above requisition is correct and just and that I have not drawn forage for any part of the time ___ ____ ____”.  Signed “Jno. A. Graham”.

“Recd at camp Feb. 29th 1864 of F. Saunders Ast Q.M. [Quarter Master] C.S. Army 5 10/56 Bus[hels] corn, 40 pounds hay, 40 pounds straw in full of the above requisition.” Signed “Jno. A. Graham”.

On the reverse is: “March 5 [5 crossed out and 12 inserted over the 5]”, then “12th, 64” written after this. ”Forage Requisition For Feb. 1864”. Signed “Jno. A. Graham”.

Requisition is for “forage for one private horse” for the period from Feb. 1st to Feb. 29, 1864. Issued Feb. 29, 1864 in the field at a camp in Virginia.

Graham was the Assistant Surgeon of the 5th Virginia Infantry and joined the 42nd Virginia as Regimental Surgeon in 1862. He served with the 42nd through the Antietam, Gettysburg, Wilderness, and Spotsylvania campaigns and in Sept. 1864. He left the 42nd on detail to the Artillery Medical Department, Army of Northern Virginia. He was promoted to Major and served on the staff of Brig. Gen. William N. Pendleton, Commander of Artillery, ANV. He was paroled at Appomattox Court House on April 9, 1865.

The 42nd Virginia saw some of the heaviest fighting in the war and was almost decimated at Spotsylvania when they lost 5 killed, 11 wounded and 210 captured in the Union assault on the Mule Shoe salient in May 1864.  [ld]
