Medal consists of a top bar that reads “VETERAN” in raised letters at center. Reverse of the bar still has its original pin.
Suspended from the bar is a striped red, white and blue ribbon which holds a circular drop.
The obverse of the drop has a 10th Corps badge at center with the letters “NYV.” Above the badge is “115” and below it is crossed rifles over a star. On the sides the badge is flanked by “1862” and “1865.”
Reverse of the medal has a bust image of the 115th’s Colonel Simeon Sammons.
