Document is pre-printed and filled out in period ink and dates from 1884. Document is black and white with a very impressive spread-winged eagle at top perched on a patriotic shield and flanked by a panoply of US flags. Under this begins a scrollwork border which along each side has three medallions showing episodes from a soldier’s life in camp and battle. Across the bottom is a large Naval battle scene within an oak wreath flanked on one side by a standing Columbia with a US flag and a Union soldier with musket wearing a rubber blanket or poncho.

At center is a large white area that reads “UNITED STATES OF AMERICA / WASHINGTON, D.C. / SOLDIER’S PERSONAL RECORD” The information is then filled in by hand for a soldier named Levi Lackey who served in Company A, 209th Pennsylvania Infantry.

The handwritten part of the document states that the 209th was engaged at Bermuda Hundred, Petersburg, Fort Steadman and Notaway Court House. There is then a short written description of the regiments service giving some minor details and at the very bottom it mentions that Private Lackey was wounded at Bermuda Hundred in 1864 and lost his right leg at Fort Stedman in 1865.

The document is housed in a modern frame and meas. approx. 19.25 x 23.25 inches. Document is in very nice condition. All ink is readable.

Levi Lackey was born in Cumberland County, Pennsylvania on August 9, 1845.

He enlisted as a Private in Company A, 209th Pennsylvania on September 6, 1864. As stated on his service document he was wounded but the record says on November 17, 1864 at Bermuda Hundred and not Fort Steadman as the document claims. He was discharged due to wounds on April 11, 1865.

He died on August 2, 1894 and is buried in Springville Cemetery, Boiling Springs, Pennsylvania.  [ad]
