Unused printed form blank issued by the Medical Purveyor's Office, Columbia, South Carolina, 186_. The form authorizes transportation for "one seat" plus a stated quantity of "sacks of Barks, etc." between two points, to be signed by the "Surg. & Med. Purveyor." Columbia served as the site of a Confederate medical purveyor's depot responsible for procuring and distributing medical supplies to army hospitals and field units, and forms of this type were used to move both personnel and scarce medical stores — likely including cinchona bark, the source of quinine, which was in chronic short supply under the Union naval blockade. Identical in format and printing to inv. #2025-2350.
This example remains entirely unfilled — no date, name, route, or signature has been entered. Toned manuscript paper, printed in a decorative script typeface characteristic of Confederate quartermaster and medical department forms. Housed in archival sleeve. [ss][ph:L]
