Printed discharge form completed in manuscript, discharging Corporal John W. Fisher of Captain Lucas' Artillery Company from the Confederate States Army. Fisher had enlisted January 1, 1862, for a one-year term, and was discharged for disability on June 3, 1862, at Camp Ingraham, on account of "Chronic Varicosele of left Side" per surgeon's certificate.

The form provides a physical description of Fisher: born in Petersburg, Virginia, age 31, 5'9", light complexion, blue eyes, light hair, and a civilian occupation as a druggist. Signed by D.W. Carroll, Colonel Commanding Brigade.

Vertical fold separations and edge chipping/loss to upper left corner, with old mounting remnants to verso; text and signatures fully legible. An interesting early-war Confederate medical discharge with detailed physical description, from a soldier whose brief service ended after only five months due to a chronic vascular condition.
